To request a service, please fill out the service request forms found below and click submit. View this PowerPoint to learn more about filling out a TTUHSC El Paso service request form.
**In order to submit the form, all required fields must be filled out completely, and the form must be digitally signed. In order to digitally sign, you must have the full version of Adobe Acrobat installed. Adobe Reader will not allow you to sign.
Other options are to print out the form and email it to researchcorelabsELP@ttuhsc.edu, or dropping off the form with your specimen during the drop-off hours at MSB, rm 2001.
